AFMC's Practice Transformation team provides individualized coaching and technical assistance to help practices succeed in PCMH and other value-based care programs. Our experienced multidisciplinary team works closely with each practice to align improvement efforts with reporting requirements and organizational goals.
AFMC's Practice Transformation Program supports Arkansas Medicaid Patient-Centered Medical Home (PCMH) practices through education, technical assistance, quality improvement resources, and peer learning opportunities. Services are provided through AFMC's Outreach & Program Support Improvement (OPSI) contract with the Arkansas Department of Human Services (DHS), Division of Medical Services (DMS).
Our goal is to help PCMH practices successfully participate in the Medicaid PCMH program, strengthen patient-centered care, improve quality outcomes, and navigate evolving healthcare requirements.

AFMC hosts regular Q&A sessions for PCMH practices to provide:
